OUR 2018 TRIP
2018 began with Barbara in Florida after spending Christmas with Laurie, JR, the girls and me in New York. For myself, I started the New Year in Honolulu with Danny. He was there to get treatment for “Peripheral “T” Cell Non-Hodgkin Lymphoma”.
January 3th, after examining Danny at the Oncologist Dr. Palalays office he was told that the cancer was a very aggressive type and needed to begin treatment immediately, today. He wanted to admit him to the hospital but there were no rooms available so he told me to take him to the ER and have him declared septic.
The ER doctors ordered and they started running tests. After some test results came in they wanted to look at his Gall bladder. He was admitted to the hospital and around 9:30pm or so he had a room on the forth floor.
The following day they planned to do a bone marrow test and test the gall bladder to see if it needs to come out before beginning chemotherapy. Along with the gall bladder problems, he also has pneumonia And did not want to talk with anyone with his constant coughing.
The next morning when I got to Danny’s room around 7:30 there were six or so Doctors and nurses working on trying to get Danny comfortable, cold shakes, then high temperature, the sweats, he just hurt all over, his heart rate was very high 153 bpm. By noon he was completing an hour or so long test on the gall bladder. Late afternoon it was decided the gall bladder would stay in place.
Other test showed the lymphoma was at stage four and he was very sick. The t-cell seems to be dividing at a high rate causing some of the discomfort? The doctors were in and out constantly checking one thing or another.
By this time is was also decided they would prepare him for chemotherapy, move him up to the sixth (Cardiac wing, no room on the seventh Oncology wing) floor,
A PICC to relplace the three IVs with two lines (one for only chemo the other for other meds) were installed in his upper arm that run into a large vain near the heart that way the chemo could be injected faster and safer. His blood was getting thinner and he may need a transfusion some time before midnight. Others would be called in to help, a heart doctor, a disease doctor etc and the ICU would be standing by to lend what assistances needed prior to, daring and post the chemotherapy treatment.
I left him early around 7:45 tonight so he could try to get some rest, because the doctors and nurses will be in and checking on his progress ever four hours and would give him a shot of steroid sometime over night
Friday started out much better then yesterday, the Oncologist Dr. Palalay came in early 6:30 or so, to see how he was doing after he received a shot of steroids late last night. The steroid they gave him is similar to some of the chemo drugs they are using for his treatment.
His bpm drop down to around 100 still high but better then yesterday. The cold shakes, the sweats were also at an minimal, no spikes of high temperature. He explained the drugs they would be using and a round of chemotherapy would last for five days, Cytoxan would take about 20 minuts, vincristine would take about 30 minuts, doxorubicin (Adriamycin) would take about 90 minutes, Etoposide Vepesid (VP-16) would be a drip over the next 24 hours
all would be administer through the PIC line in his right arm, Prednisone would be by mouth. Day two consisted of four chemicals; vincristine, doxorubicin (Adriamycin), Etoposide, Vepesid (VP-16) and Prednisone. Day three consisted of three chemicals; Vepesid (VP-16) and Prednisone. Day four and five only one chemical; Prednisone, and what they did to the T-cell to kill and dismantle them also the need to monitor his blood, kidneys, heart and things that is affected by chemotherapy.
As he explained the dismantling of the T-cells could overwhelm the system in removing the waste from his body, and how day one treatment with five drugs hits the lymphoma hard. By 7:30pm the first round of chemotherapy had started.
I didn’t get to Dannys’ room until a little after 8:00, he was eating breakfast surprise, surprise. If you didn’t know he was sick you couldn't tell by the way he was today, He was working on his computer checking emails, and communicating with the people on Kwajalein it has some personal item sent here that he needed.
Three of the four drugs in his “day one” treatment was finish and the forth was on the 24 hour drip. Day two” treatment has three drugs, one for about 30 minutes and one will take about 90 minutes to complete before starting the drug that drips for 24 hours. About 9:30 tonight they will be getting his "day two" treatment ready.
Sunday was a good day for Danny, he had breakfast and dinner, no lunch he was just not hungry. He still has some water retention below his chest all the way to his toes so they upped his “Lasix” input.
One of the heart Doctors came in early today and said they could removed his heart monitor because the blood test continues to look good.
His surgeon came in later today and informed him they needed to install a more permanent access known as a “port”. The “port” will replace the two line “PICC” that was put in his upper right arm for his chemotherapy treatment. The “PICC replaced all three “IV”s installed whn he first got in the hospital, two in the lower left arm and one in the lower right arm.
As for "day two" treatment, it went well and “day three” will start around 11PM Sunday night with only one drug that should only take 90 minutes, then the chemotherapy for this
first round of treatment is over.
Monday the 8th, today he will have a “Port” installed in his upper right chest to replace the PICC in his right arm. Having stopped all IVs around noon, so they could installed the “port” later today, he was out of the OR by 7:30pm and doing well. Two days later they removed the two line PICC all went well. He would stay in the hospital until his blood count recovered from the chemotherapy, will that took another week and he was discharged on Wednesday the 17th.
Thursday just one day out of the hospital, Danny was doing as well as could be expected, “O2” levels good (93-97), “HR” was higher than it should be (93-120), "BP" was low (82-102/52-66), his temperature was normal.
Friday the 19th was much the same except his legs were starting to hurt with red splotches which continued to get worst as the day went on.
Saturday his legs turned mush worse, the pain was so great he could not touch them. Claritin and Tylenol did not help at all so by 1:30pm I got a wheelchair and took him to the “ER”. A soon as he got checked in and the “ER” doctor seen him they gave him some morphine for the pain.
To check out the red splotches they called in a dermatologist DR Johnson who did a biopsy on his wrist area the only red splotch that was not on his legs. Dr Johnson after some research on line said it looked like "Trousseau's Syndrome" the first case he had ever seen in his thirty-five years of practice and they gave him some 40MG of Prednisone (steroid), they also gave him 5MG of Oxyicodine for pain. He stayed in the hospital until they determined that Prednisone alleviated the red splotches.
So late Monday the 22nd he was discharged and went back to Hope Lodge.
He would take the steroids until the cancer was no longer influencing the formation of migratory thrombophlebitis
The next three days at Hope Lodge he was resting and his vitals continued to get better. On the afternoon of the 26th he checked back in to the hospital so he could start the second round of chemotherapy that night, the chemotherapy would be the same as the first round.
After completing the three days of chemotherapy he was discharged on the 29th and back to Hope Lodge. He would have to make a trip to the hospital to get a shot of Zarxio for the next five days to help build up his white blood count.
On the 9th of February he went for his last office visit to Dr Palalay before flying to Florida. Dr Palalay gave him additional oxyicodine for some pain relief and Prednisone-steroid that helps prevent formation of migratory thrombophlebitis to last him until he sees the doctor at Moffitt on the 14th of February.
The night of Febuary 9th we boarded a flight for Houston. the flight that night was around 9 hours or so long and through 4 time zones before landing just at day break. There was maybe three hours or so layover before boarding our next flight to Tampa.
Danny had an appointment at Moffitt on the 14th with Dr Sokol where he scheduled a time for his next round of chemotherapy. After checking in to Moffitt on the 16th his third round was started. The chemicals were basically the same except for the 24 hour drip was replaced with a 15 minute shot. With this round of chemotherapy completed and his blood count coming back to normal they discharged him on the 26th of February.
Danny had an appointment at Moffitt on the 14th with Dr Sokol where he confirmed that Danny would receive a total of three types of chemotherapy: the first series (six rounds-two rounds he has already received in Honolulu with the remaining four rounds here at Moffitt) that would be to treat/attack the cancer in the blood/limp system, the second series (two rounds) would address the brain and the third would be in the Bone Marrow Transplant (BMT) clinic.
By the end of the appointment with Dr. Sokol he had scheduled his next round chemotherapy that would start on the 16th.
After checking into Moffitt Cancer Center they started his third round of chemotherapy.
The chemicals were basically the same except for the 24 hour drip was replaced with a 15 minute shot. With this round of chemotherapy completed and his blood count coming back to normal they discharged him on the 26th of February.
Danny had his fourth and fifth rounds of chemotherapy on schedule, every twenty-one days like all the others. Between the fourth and fifth round of chemo he had a PET scan to see if the cancer is in remission and it was.
Before his next round we all took a trip up to Bennett North Carolina for Whitleys first birthday. We made it back home just in time for Danny to have his sixth round of chemotherapy.
